Importance of emergency planning and drill for OHSAS 18001 standard implementation
February 18th, 2012
ISO Certification Consultants Emergency Planning to deal with situations is an important element of OHS Management System.
Emergencies can occur at any time which gives rise to the need of rapid decisions, resources and trained personnel. If there is a lapse in any of these, including time, it can lead to chaos and stress which leads to poor judgment resulting in severe losses.
Preplanning prevents possibility of disaster. It provides the major benefit of providing guidance during an emergency.
By developing an Emergency Response plan, one indentifies the unrecognized hazardous conditions that would aggravate an emergency situation and indentifies and implements ways and means to eliminate them.
The planning process brings to light deficiencies, such as the lack of resources (equipment, trained personnel, supplies), or items that can be rectified before an emergency occurs. It specifies procedures for handling sudden unexpected situations.
Completing a comprehensive plan for handling emergencies is a major step toward preventing disasters. However, it is difficult to predict all of the problems that may happen unless the plan is tested.
Drills have be conducted to practice all or critical portions (such as evacuation, caring of the injured.) of the plan. A review after will point out areas that require improvement.
The Drills may throw light on the shortcomings of the entire response process, which can be reviewed and changes could be made to the existing plan. Need for training to properly respond to emergency situations may also be identified.
In conclusion an emergency plan promotes safety awareness and shows the organization’s commitment to the safety of workers and a properly conducted drill helps maintain a high level of proficiency.
Measurement Uncertainty
February 18th, 2012
ISO Certification Consultants Measurement Uncertainty
For the uncertainty to be truly meaningful, it must address the entire measuring process, which may have uncertainties associated with factors such as equipment calibration, equipment resolution, operator skill, sample variation, and environmental factors. In many cases, sample variability and operator skill are the largest sources of uncertainty and they are often the only source considered when only the repeatability of a measurement is evaluated. However, a more thorough analysis considers other sources of uncertainty. As a minimum, in addition to the repeatability evaluation, an uncertainty analysis should consider the instrumentation/standard calibration and the resolution of the instrumentation.
The following a summary of the main steps in an uncertainty evaluation.
- Define the measured. (Determine exactly what needs to be measured.)
- Carry out the needed measurements.
- Evaluate the uncertainty due to the calibration standard and/or instrumentation used for the measurement.
- Evaluate resolution/readability of all instrumentation.
- Evaluate the repeatability.
- Identify and evaluate other sources of uncertainty.
- Prepare uncertainty budget documentation.
- Evaluate reasonability of budget.
- Determine combined standard uncertainty.
- Express the uncertainty in terms of uncertainty interval and the confidence level.
The first step controlling and characterizing uncertainty in a measurement is to define the measurand as explicitly as possible. It is very important to know exactly what requires measuring. If possible, determine the required level of precision and accuracy and whether they can be obtained with the available equipment. Verify that equipment is properly calibrated and/or perform user calibration check using NIST or ISO traceable reference standard. If systematic error (bias) is found to exist, record the value so that it can be eliminated with a correction to the measurement data. It is also important to ensure that test samples are identified to prevent mix-ups and to mark measurement locations to allow for repeat measurements of the same locations. Finally, make the measurements with care and correct for any bias that has been identified.
To evaluate sources of uncertainty due to factors such as sample variability, placement of the measurement instrument, and operator skill and consistency in making the measurement, a repeatability study should be conducted. A repeatability study is only useful when the measurement device is sensitive enough to produce scatter in the readings. Repeat readings that all produce the same value may improve overall confidence in the measurement, but they don’t provide any additional information about the probability density function. Therefore, the measurement must be sensitive enough to produce scatter in the reading so that the shape of the distribution can be determined and the standard deviation can be properly calculated. When repeat readings produce scatter that is distributed approximately equally about the mean value of the sample.
An uncertainty budget is simply a way of organizing and summarizing the uncertainty analysis in tabular form. An uncertainty budget lists all the contributing components of uncertainty and these components are used to calculate the combined standard uncertainty for the measurement.
Other source of uncertainty will likely be small compared to the instrumentation and repeatability evaluations discussed above, but all identifiable sources of uncertainty should be addressed. Other possible sources of uncertainty may include: parallax, thermal expansion and other temperature effects, voltage drift and etc.
Once the standard uncertainties for all the sources of uncertainty in a measurement or set of measurements have been calculated, then the combined standard uncertainty can be calculated. The combined standard uncertainty is the total uncertainty in the measurement and can come from a combination of type A and B evaluations.
The results of the measurement and uncertainty analysis should be reported in terms of the uncertainty interval and the confidence level.
Uncertainty contributions from both Type A and Type B evaluations may be combined as long as they are expressed in similar terms before they are combined. Thus, all the uncertainties must be expressed as one standard uncertainty and in the same units.
For the uncertainty to be truly meaningful, it must address the entire measuring process, which may have uncertainties associated with factors such as equipment calibration, operator skill, sample variation, and environmental factors. When a measurement has more than one identifiable source of measurement uncertainty, then the combined standard uncertainty (uc) must be calculated.
Many physical quantities are not determined from a single direct measurement but instead are calculated by combining two or more separate measurements. Therefore, it is important to understand how measurement uncertainty propagates when mathematical operations are performed on measured quantities, so that a final combined uncertainty can be calculated.
The equations in the table above or only valid if the contributing uncertainties are not correlated. Factors leading to measurement error are often independent, but sometimes they are correlated of inter-related.
An uncertainty budget is simply a way of organizing and summarizing the uncertainty analysis in tabular form. An uncertainty budget lists all the contributing components of uncertainty and these components are used to calculate the combined standard uncertainty for the measurement.
Clause no 6.2.2 of the ISO 9001: 2008 standard specifies the following requirement
February 17th, 2012
ISO Certification Consultants Clause no 6.2.2 of the ISO 9001: 2008 standard specifies the following requirement:-
“The organization shall
a) determine the necessary competence for personnel performing work affecting conformity to product
requirements,
b) where applicable, provide training or take other actions to achieve the necessary competence”
In order to meet this requirement, the organizations must understand what is the meaning of the word “competence” and how to carry out competence evaluation.
Competency is defined as an individual’s set of skills and abilities that are measurable and observable. These are categorized into three: functional, managerial, and behavioral. It could be a cumulative effect of characteristics, knowledge, skills, and capabilities that, when utilized, would result to an effectual performance. It is a means by which businesses and organizations can work towards their goals. In addition, as a rule of thumb, they should be able to identify the various competencies of their employees, from upper management to the front liners.
Competencies include the collection of success factors necessary for achieving important results in a specific job or work role in a particular organization. Success factors are combinations of knowledge, skills, and attributes (more historically called “KSA’s”) that are described in terms of specific behaviors, and are demonstrated by superior performers in those jobs or work roles. Attributes include: personal characteristics, traits, motives, values or ways of thinking that impact an individual’s behavior.
Competenceis a standardized requirement for an individual to properly perform a specific job. It encompasses a combination of knowledge, skills and behavior utilized to improve performance. More generally, competence is the state or quality of being adequately or well qualified, having the ability to perform a specific role.
The competencies have five characteristics, namely:
* Motives: Things a person consistently thinks about or wants that cause action, motives drive, direct and select behavior towards certain actions. Example achievement motivation people consistently set challenging goals for themselves, take responsibility for accomplishing them and use the feedback to do better
* Traits: Physical characteristics and consistent responses to situations. Good eyesight is physical traits of a pilot. Emotional Self Control and initiative are more complex consistent responses to situations.
* Self Concept: A person’s attitude value or self image. A person’s values are reactive or respondent motives that predict what a person would do in the short run. Example: A person who values being a leader would be more likely to exhibit leadership behavior.
* Knowledge (Information a person has in a specific work area) Example: An accountant’s knowledge of various accounting procedures.
* Skill (is the ability to perform certain mental or physical tasks) Example: Mental competency includes analytical thinking. The ability to establish cause and affect relationship.
COMPETENCY MAPPING
Competency mapping is a process through which one assesses and determines one’s strengths as an individual worker and in some cases, as part of an organization. It generally examines two areas: emotional intelligence or emotional quotient (EQ), and strengths of the individual in areas like team structure, leadership, and decision-making. Large organizations frequently employ some form of competency mapping to understand how to most effectively employ the competencies of strengths of workers. They may also use competency mapping to analyze the combination of strengths in different workers to produce the most effective teams and the highest quality work.
What is a Competency Matrix?
A competency matrix is a tool by which a person’s competencies are assessed. It is used as a means to define the required competencies for a particular position, as well as a tool to help identify the most suitable individuals for the job. As mentioned, it is categorized into three, which are: functional, managerial, and behavioral.
Functional: This refers to the technical competencies or the hard skills that a professional would need in order for him to perform his job. This varies of course, depending on the job. For example, an IT professional should be skilled at Java Development and PHP programming; a management accountant should be good at economics, corporate finance, and financial management.
Managerial: This refers to one’s ability to work productively with other people. This can be classified into: organizational, people-related, and role-related. This includes several aspects such as, time management, staff developing, decision-making, delegation, communication, and resource management.
Behavioral: This refers to the interpersonal and “people” skills of an individual. This is where a person’s soft skills are categorized. Such skills include leadership, teamwork, motivation, adaptability, communication, service-oriented, initiative, and dynamism, among others.
How to Make a Competency Matrix
A competency matrix may be customized depending on the kind of organization or business you are in. But these are the general guidelines:
1. Develop a list of all the required competences you need in order for your organization to run successfully. Of course, these have to be aligned with your organization’s objectives, vision, mission, as well as strategies.
2. Based on your list of competencies, classify them as to the following: functional, managerial, and technical. Once you have done this, identify the level of proficiency. This can be in the following format:
- Beginner
- Learner
- Skilled
- Expert
Define these proficiency levels based on your organization’s requirements.
3. Then, as soon as you have the categories and levels of proficiency identified, determine the proficiency level of each competency required for a particular job position. This should be done for all positions in your organization, and this process is otherwise referred to as competency mapping.
Competency mapping is a process that is used alongside the competency matrix. In essence, the matrix is the tool and the mapping is the process. Once you are able to identify the level of competencies of each individual, you can then pinpoint which of these competencies are required to perform successfully a job or a role.
Risk management is an increasingly important business driver and stakeholders have become much more concerned about risk
February 17th, 2012
ISO Certification Consultants Risk management is an increasingly important business driver and stakeholders have become much more concerned about risk. Risk may be a driver of strategic decisions, it may be a cause of uncertainty in the organization or it may simply be embedded in the activities of the organization.
An enterprise-wide approach to risk management enables an organization to consider the potential impact of all types of risks on all processes, activities, stakeholders, products and services. Implementing a comprehensive approach will result in an organization benefiting from what is often referred to as the ‘upside of risk’
ISO 31000:2009 provides generic guidelines for the design, implementation and maintenance of risk management processes throughout an organization.
It is a framework that can be integrated across various industries and regions and adopted by any organization – including public, private, not-for-profit and government organizations.
The scope of this approach to risk management is to enable all strategic, management and operational tasks of an organization throughout projects, functions, and processes to be aligned to a common set of risk management objectives.
ISO 31000 was published as a standard on the 13th of November 2009, and provides a standard on the implementation of risk management. The purpose of ISO 31000:2009 is to be applicable and adaptable for “any public, private or community enterprise, association, group or individual. Accordingly, the general scope of ISO 31000 – as a family of risk management standards – is not developed for a particular industry group, management system or subject matter field in mind, rather to provide best practice structure and guidance to all operations concerned with risk management.
Application of ISO 31000
ISO 31000:2009 provides generic guidelines for the design, implementation and maintenance of risk management processes throughout an organization. This approach to formalizing risk management practices will facilitate broader adoption by companies who require an enterprise risk management standard that accommodates multiple ‘silo-centric’ management systems.
Accordingly, ISO 31000:2009 is intended for a broad stakeholder group including:
- executive level stakeholders
- appointment holders in the enterprise risk management group
- risk analysts and management officers
- line managers and project managers
- compliance and internal auditors
- Independent Practitioners
Benefits of ISO 31000 – Risk Management
For all types of organizations, there is a need to understand the risks being taken when seeking to achieve objectives and attain the desired level of reward. Organizations need to understand the overall level of risk embedded within their processes and activities. It is important for organizations to recognize and prioritize significant risks and identify the weakest critical controls.
When setting out to improve risk management performance, the expected benefits of the risk management initiative should be established in advance. The outputs from successful risk management include compliance, assurance and enhanced decision-making. These outputs will provide benefits by way of improvements in the efficiency of operations, effectiveness of tactics (change projects) and the efficacy of the strategy of the organization.
A successful risk management initiative can affect the likelihood and consequences of risks materializing, as well as deliver benefits related to better informed strategic decisions, successful delivery of change and increased operational efficiency.
Other benefits include reduced cost of capital, more accurate financial reporting, competitive advantage, improved perception of the organization, better marketplace presence and, in the case of public service organizations, enhanced political and community support.
Risk management is a process which provides assurance that:
- objectives are more likely to be achieved;
- damaging things will not happen or are less likely to happen;
- beneficially things will be or are more likely to be achieved.
It is avoiding risk. not a process for The aim of risk management is not to eliminate risk, rather to manage the risks involved in all University activities to maximise opportunities and minimise adverse effects.
Good risk management provides upward assurance from business activities and administrative functions, from department to faculties, to the senior management team and ultimately to the governing body.
